Community Project
Mary Stevens Hospice is currently inviting feedback from the community it serves, to identify ways in which access to its specialist palliative care services could be further enhanced.
The project aims to help Mary Stevens Hospice work with others, to better support the people of Dudley and neighbouring boroughs with life-limiting illness, as well as their friends and families.
The main aims of the project are to gather views on:
· Access to specialist help and support
· Type of care/support needed, particularly:
· Outside of office hours(ie Monday-Friday, 9 am – 5 pm) and, in particular:
· Care in the final part of a person’s life
Mary Stevens Hospice has a team of skilled staff, experienced in palliative care, dedicated to helping patients, not only with practical nursing care but also the spiritual and emotional support which is so invaluable. We help adults (aged 18 or over) cope with the impact of life limiting illness.
Until January 2012 we are seeking the views of patients, relatives, carers, health workers, voluntary workers and social service providers to help us shape future developments.
We would be very glad to know your views and appreciate you taking the time to complete the accompanying survey. Please return the questionnaire by the 30th November 2011.
You can find the questionnaire Here: Community Questionnaire.docx
Many thanks for your help.
